East Riding of Yorkshire Clinical Commissioning Group (CCG) is delighted to be shortlisted in the HSJ Awards for the Primary Care Innovation of the Year award.
The award recognises those schemes that demonstrate innovative ways of supporting people to stay out of the hospital and receive care closer to home.
Since 2017, the CCG has been actively working towards increasing the opportunities to support East Riding of Yorkshire residents living with conditions affecting joints, ligaments, tendons, muscles and nerves through the introduction of a Musculoskeletal (MSK) programme.
The aim of the programme is to help treat people in their local community much quicker, through specialist clinicians in their GP practice along with courses that offer supported self-management.
The programme brings together a number of different partners: consultants, general practitioners, physiotherapy services, Health Trainers from East Riding Leisure Centres and Versus Arthritis.
By working together clinicians are able to make better decisions in managing their patients’ conditions and patients have easy access to independent expert advice on their condition, physiotherapy sessions and tailored exercise programmes.
Ulf Clausen, local GP and musculoskeletal conditions lead for East Riding of Yorkshire CCG said;
“We have a clear vision, to create an environment where patients can understand their condition better, become confident in managing their symptoms and ultimately have the ability to identify what their bodies are capable of doing despite their ailments.”
“For patients, the rewards are tremendous, with many reporting major health improvements from simple tasks such as ‘putting their socks on’ themselves, being able to climb stairs, walk further and feel good about themselves, and most importantly the ability to manage their pain and reduce their medication.”
Dr Anne Jeffreys, Local GP and East Riding of Yorkshire CCG Chairperson said;
“The CCG is very pleased this initiative has been recognised as a worthy nomination for the HSJ Primary Care Innovation of the Year Award.”
“This is testament to the hard work, dedication and enthusiasm of Ulf Clausen, our Primary Care Team and our partners who continue to look at new and innovative ways of improving the health and wellbeing of our East Riding residents.”
Winners will be selected ahead of the ceremony, which will take place virtually in March 2021.
